SF1-2 (01/2013)	Salesperson Sponsorship Form-1 – Use When Changing Brokers or going from Inactive to Active or renewing an expired license that has been expired less than six months and will be sponsored by a new broker.
SF2-1 (09/2011)	Salesperson Sponsorship Form-2 – Applying for sponsorship for the first time
